What is Pelleve’?
Pelleve’ is the most significant advance in
nonsurgical skin tightening available. Using
continuous, high frequency, radio wave
energy (RF), Pelleve’ generates heat in the
skin, which causes it to contract. Aging and
sun damage cause the collagen in the skin
to become less dense and less tightly bound.
Fibrous bands that hold your skin firmly to
the muscle also weaken, causing the skin to
lose definition and sag. Pelleve’ heats the
collagen to a point that it can release and
recontract, resulting in tighter skin without pain.
How does it work?
Pelleve’delivers constant, gradual energy,
causing heat to build up where the skin and fat layer come
together. The increasing heat near the skin’s surface denatures, or
modifies, the collagen protein bundles, causing them to contract
horizontally. This results in denser, tighter skin. The increasing
heat in the deeper layer contracts the fibrous bands that run
through the fat layer and pull the skin closer to the underlying
muscle, causing vertical contraction. The skin is then rapidly
cooled to force further contraction. The key is to deliver the
amount of energy it takes to trigger maximal contraction for each
individual patient, as opposed to a predetermined, fixed dose of
pulses.
Does Pelleve’ burn the skin?
No. A nutrient-enriched dispersion gel is applied to the skin
throughout the procedure, keeping the surface of the skin cooler
than the deep dermis where the heat is concentrated. At the same
time the energy drives nutrients, such as hyaluronic acid, DMAE
and phospholipids, into the skin as the hydrating gel is absorbed.
These nutrients are essential for skin rejuvenation. In the deeper
tissue, the temperature is gradually and painlessly raised to the
point that collagen can denature and contract.
If I need filler, why should I get
the area treated with heat?
If you have a visible wrinkle line,
crease or groove, filler will help. But
those wrinkles mean your skin has lost its
integrity, firmness, thickness and elasticity
and needs to be replenished first. Hard
V-shaped lip lines, for example, can be
softened to a soft U-shape with Pelleve’.
This leads to a smoother look once filler
is injected. Nasolabial folds, glabellar lines
and marionette lines can be reduced to
a soft, shallow groove by reducing the
thickness of the high side of the fold.
Crow’s feet can be softened by flattening
the high ridges and firming the low areas
of the creases.
Used before overall volume restoration with
Sculptra® or Radiesse®, Pelleve’ compresses the subdermal plane
to achieve firmer results with less filler. Dr. Stampar always heats
first and then uses the appropriate filler.
Does Pelleve’ affect filler or other treatments?
What about colored skin?
Tests show there is no harm in RF heating over fillers that
are currently available for use. As artificial pigments can be
affected by heat treatment, care is taken to not heat directly over
permanent make-up. Although Pelleve’ was only cleared for the 4 lightist skin types, RF energy is blind to skin type and color in all other studies and that fact is replicated in my personal experience.

Is there anyone who can’t get Pelleve?
Other than pacemakers and implanted defibrillators more than 20 years old, there
are no medical contraindications to Pelleve’. Many patients who
cannot have a surgical procedure can benefit from nonsurgical
skin tightening.
